Obtain An Assisted Living License

Most assisted living facilities across the nation need to be licensed by the state agencies that monitor them. To obtain and retain a license, a facility has to meet certain standards, most importantly in regard to resident care. Obtaining the license usually involves several steps.


Instructions


1. Submit a completed application with the required fee to the licensing agency in your state, usually the state Department of Health, Human Services or Aging. License fees vary but generally consist of a basic amount, plus a fee for each bed in the facility. Fees must be paid when the application is submitted.








2. Provide a copy of your facility's policies and procedures, and a copy of the resident agreement. Give the name of the owner, or the names of each general partner or officer if the facility is organized as either a partnership or corporation. The policies must be established by a team of professional personnel and are intended to regulate the services provided by the facility. Other information needed includes the physical address of the facility, legal mailing address and telephone number. The application will ask you to specify the maximum number of beds available in the facility.


3. Indicate the name of the chief administrative officer and that person's qualifications. If evidence of the administrator's qualifications is required, submit proof of the individual's education and training. Certifications and membership in professional organizations may be helpful. You must also attach a list of the organization's board members, their occupations, contact information and positions on the board.


4. Show that the building in which the assisted living facility is located meets local fire code ordinances and has passed an inspection conducted by the local fire marshal. The physical structure must comply with all health and safety codes and regulations. The operation of the facility must also meet the licensing standards of that state's Department of Health or Public Welfare.








5. Attach a list of the services that will be offered to residents of the facility. Be sure to submit any other required documentation along with the original copy of the license application. Other documentation that you may need to submit includes a copy of a partnership agreement or certificate of incorporation, lease agreement or deed of ownership of the facility.
